file manager popup bug

Dunno if anyone else found this one or not, but I have a bug in the file manager but only when it is launched via the "Files" button. In other words, going through the "Files" menu shows me the files like I would expect, but when I am editing a post and click "Files" I get thrown an error:
An unexpected error has occurred!
If this error persists, please report it to the administrator.

Additional information about this error:

MySQL error!

Duplicate entry 'collection-2-http_BL_plugin.zip' for key 2(Errno=1062)
Your query: DataObject::dbinsert()
INSERT INTO qp_files (file_root_type, file_root_ID, file_path)
VALUES ('collection', 2, 'http_BL_plugin.zip')

I uploaded all the files via FTP, so they have no title or alt or anything. Via Files tab it works like you'd expect. Via Files button I get the above. It was okay on maybe 12 files before this, and all of these files have been "attached" successfully.

Dunno why it is trying a dbinsert() ???

Which version? Of course I embedded the file manager so I need to do a fresh install.

I had some strange file manager problems due to settings in my .htaccess file. I was trying to prevent spam. The thing is, they should not have throw any errors in the file manager but they did. AND I only saw them in the pop-up file manager, not the regular file manager.

Oops. QP5 so now I need to do a QP install, "FTP" the same files over, and see if I have the same problem.

Just tried this on QP and the QP5 (qp5_v14067) install I have on my dev machine and both worked fine.

Just remember this incident I had with file manager. I was getting a MySQL error with the file manager in a pop-up but not the regular. Ed suggested that it was a duplicate file name but one would think that would also trip up the regular file manager. I went to the database using phpMyAdmin and could not see a duplicate file. However, I deleted the file in question and re uploaded it and eventually resolved the problem. In my case this happened after I migrated the blog from b2evo to QP, so I thought a glitch got introduced somewhere. I also lost all the file dates for the media folder; all of my images have the same date; something I would like to fix but fear corrupting the database.
